Outpatient Admissions
How To Start
If visiting RCH for an outpatient service, first visit RCH’s Admissions area to complete any necessary paperwork. This includes the Authorization and Consent for Treatment Form, which must be completed upon each visit and gives RCH permission to complete the services ordered by your physician. You will also need the following information:
- A list of the medication(s) you’re currently taking.
- Current medical insurance cards or insurance coverage information.
- Photo identification.
- A copy of your Living Will or a Durable Power of Attorney for Health Care Decisions if it is not already on file with RCH.
If you receive Medicare, you will be asked Medicare-related questions during your registration.
If you are receiving treatment for a work-related accident, auto accident or an accident that occurred on someone else’s property, please bring any information you already have that may help us contact the responsible party.
Pre-Registration
RCH also offers pre-registration for any patient who knows they will be coming to the hospital for outpatient services. If you’re interested in this option, please call the Business Office at 785-434-4553 and asked to be pre-registered.
To pre-register, you must know the following:
- Date of your visit
- Services you’ll be receiving
- Name of the physician who ordered the service
