Supporting Patients Through the Transition Home
After a hospital stay, many patients aren’t quite ready to return home, but they also don’t need the intensity of acute inpatient care. What they do need is time, skilled support, and a clear plan for recovery.
At Rooks County Health Center, our Swing Bed program provides that next step. We offer hospital-based skilled nursing and rehabilitation services designed to help patients regain strength, rebuild confidence, and transition safely back to everyday life.
What Is Swing Bed Care?
Swing Bed care allows eligible patients to remain in a hospital setting while receiving skilled nursing or intermediate care following an acute stay.
This level of care is often appropriate for patients who:
- Are medically stable but need continued monitoring
- Require skilled nursing care or rehabilitation
- Would benefit from additional time and support before returning home
Swing Bed services are covered by Medicare and serve as a valuable bridge between hospital care and home.
Comprehensive Care, All Under One Roof
One of the greatest strengths of RCH’s Swing Bed program is access. Patients receive coordinated care without leaving the hospital environment.
Our Swing Bed services include:
- 24/7 skilled nursing care
- On-site Occupational Therapy, Physical Therapy, and Speech Therapy
- Access to Cardiac Rehabilitation when appropriate
- Close collaboration between nursing, therapy, and medical providers
This integrated approach allows care teams to adjust quickly, communicate clearly, and focus on steady progress toward recovery.
A Program Designed for Patients, Families, and Care Teams
For patients and families, Swing Bed care at RCH offers reassurance during an often uncertain time. Care is delivered in a quieter, more personal hospital setting, where patients are supported by a team that values consistency and communication.
For referring hospitals, our Swing Bed program provides:
- A reliable option for continued skilled care
- Support for smooth transitions and discharge planning
- Ongoing coordination with your care team
The program is led by Michele Hinger, Swing Bed Coordinator, who works closely with referring facilities, patients, and families to ensure a seamless experience from referral to discharge.
Partnering With Hospitals Across the Region
RCH is proud to serve as a Swing Bed resource for hospitals throughout the region. Our team welcomes collaboration with facilities seeking high-quality post-acute care options for their patients.
If your hospital has patients who would benefit from hospital-based Swing Bed care, we are ready to help.
